If you child or child's friend is interested in participating in our ministries here at St. Luke, please download this form, fill it out (including the notarization) and bring it back to the office.  It is good for an entire school year and allows full participation in all the great events we have going on here.

Interested in Volunteering?

We are always on the lookout for gifted individuals who are called to work with the formation and discipleship of our children.  If you are interested, please contact the church office and set up a time to get together with the youth or children's minister.
